Erhard's Tomb
a sealed sarcophagus, a waxy man, a ruby for a heart
Floor covered in drifting fog. Stale, warm air. Dark. Inside, a standing sarcophagus (sealed), flanked by two unlit torches. "Tunnels lead west to a ruin-tunnel from The Plague Pit, north to a narrow crawl space to the Catacomb Corridor, east to a tunnel to the Plague Pit, south to a tunnel to the Colander Room." Every direction leads somewhere worse, and the room exists only for the box at its centre.
Opening the Sarcophagus. Test Strength DR18, reduced by 2 for each assistant. Failure means everyone involved takes d4 damage as the lid falls open. Inside lies Erhard — "a man's odorless corpse in near perfect condition, wearing fine silk clothes. The skin is waxy, malleable like dough and very flammable. Digging into the body reveals an iron skeleton, a fist-sized ruby inside the ribcage."
A tomb that someone built with great effort, around a body someone else prepared with greater. Both are nameless. Only the tomb has a name, and it is the name on its door.

Whether the unlit torches are unlit because no one lights them, or unlit because they refuse to take a flame, is unwritten. Whether the seal of the sarcophagus is the work of the same hands that made the iron skeleton is unstated. The west ruin-tunnel from the Plague Pit suggests the chamber predates the rest of the graveyard's tunnel system — Erhard was here first.
