The Death Ziggurat
Deep in Sarkash an unnaturally cold depression yawns in the forest floor — a sinkhole seemingly formed by the strike of an impossibly large fist. Within, ancient ruins rise among the trees. Murky clouds swirl down into the depression like a tornado. Hidden in the fog at the bottom, covered in icy sludge, is the ziggurat.
The ziggurat exists in two places: a barren waste in the living world (swarming with undead), and the actual ziggurat in the Death Realm, reachable only by a Rot Priest's medallion. Inside is Akünh, the demon-child spawn of SHE and a demonic being from the Death Realm. She was bound here by knights of the Creton Order centuries ago. Now the old shackles have failed; she has broken free to muster her undead army before the world ends.
Sites around the depression include:
- The Frozen Lake of black flowers that touch with strange permanent effects
- The Spiral Chapel dedicated to Heedless Creation, hiding the Spiral Crown
- The Necropolis in the Death Realm, with its bottomless pit from which Horn Beasts crawl
- The Rot Priest camp of conical incense-smelling tents
- The death-obsessed Cultist camp half an hour's walk from the depression
- A foul-smelling cave that opens into a rotting mouth in the woods, sealed unless blood drips on it
If Akünh is undefeated she will conquer the closest castle and use her horde to ravage the lands. If her heart is destroyed she is banished into the void until the end of days — but this instantly causes one Misery to activate.

The depression's exact location in Sarkash is left as "deep in the woods." Akünh's demonic parent is unnamed. The "closest castle" she would conquer if undefeated is GM-pick. The deep cities of the toad-people (referenced in Sepulchre of the Swamp Witch) may be other Death-Realm-adjacent sites worth linking.
