A long, curved blade shaped like a tongue, buried beneath the sludge of the Sewage Pit where the Fleshy Automatons deposit Silas' collected filth. "The only thing Silas can't digest is metal." Beneath the muck: d6 silver and this knife. Found on a careful search or a Presence DR14; the same search may trigger the chamber's falling block (d6 damage).
Damage: d6. Curse: "On a hit, the target becomes ravenously hungry and desperately thirsty — they must eat and drink within the hour or die."
A relic that compels the same hunger the Gourmand's Cutlery compels in its wielder, but pushes it through a stranger's flesh instead of one's own. A piece of the chapel-cult's worship that someone — possibly Silas, possibly a priest before him — swallowed by accident or by rite, and never passed.

Whether the knife is one of a set (paired with the Cutlery, perhaps — a fork, a knife, a spoon) is unstated. Whether the compulsion to eat-and-drink can be satisfied by anything (a leaf, a swallow of swamp water) or requires a proper meal is GM-call. Whether the target dies of metaphysical hunger, or of literal starvation collapsed into one cruel hour, is canon-silent.
